NAME: CHARLIE SIN DATE OF BIRTH: November 22, 1995 PLACE OF BIRTH: Hong Kong DATE ADMITTED: August 8, 1996 Charlie is an abandoned child who was diagnosed to have congenital hydrocephalus. The child had undergone an initial operation in Hong Kong but due to shunt malfunction, he had to undergo a second operation on June 1996 at the East Avenue Medical Center. Charlie was four months old when his biological mother left him to the care of a close friend with the intention of getting the child back after three days. However, the mother never came back. His case was reported to the social welfare system. Charlie was referred to and admitted to Chosen Children on August 8, 1996 because he needs proper medical attention and case management. Upon admission, Charlie was observed to have a good socialization pattern. He seems to be adjusted to his new environment; he is easy to feed and has a good sleeping pattern. He was also brought to a Pediatrician at the Makati Medical Center, who diagnosed Charlie to have developmental delays. Spasticity was noted on both his upper and lower extremities and he was found to have cerebral palsy. |
