NAME: Charmaine Anne Mendez DOB: December 4, 2000 BIRTHPLACE: Quezon City(given) Special Needs: Congenital Heart Disease/Glaucoma/Rubella Charmaine Ann is a 9 months old baby girl allegedly born on December 4, 2000 in Quezon City. The child case study report stated that a concerned citizen found the child crying on top of a plastic bag wearing a white dress and white pajamas. Immediately, she took the child and reported it to the police station in Quezon City who subsequently referred the said child to the social welfare system. She was referred to Chosen Children, for proper case management and appropriate medical attention, where she was admitted on August 14, 2001. Charmaine was diagnosed to have complicated medical problems such as congenital heart disease, congenital glaucoma (eye problem) and congenital rubella (virus). At the time of admission to Chosen Children, she also had coughs and a cold. Thus, she was brought for assessment to a Pediatrician at the Makati Medical Center. Findings revealed the child has upper tract respiratory infection. Since the child has congenital rubella, the doctor advised that proper handling of the child’s stool is needed because it discharges viruses and is therefore infectious. The child also has heart murmur according to the assessment of a Cardiologist at the Makati Medical Center.
